Your medical records are confidential and protected by federal privacy and security regulations, and will be automatically transferred in a HIPAA-compliant manner to the Texas Sinus & Respiratory Disease Center. 

If you require medical records be transferred elsewhere, please complete the medical records release authorization form (click the three dots icon in the top right corner to download) and submit as an attachment to a new message in the patient portal. Your patient portal account is associated with this email address. If you have issues submitting the request in the portal, please call our office at (214) 821-1809 and leave a voicemail on extension 1.
